Nutritional therapy in malnourished young adolescent: from epileptic state to catch up growth
Abstrak
Epilepsi dan malnutrisi pada anak merupakan lingkatan setan. Epilepsi dengan kejang berulang yang tidak diatasi dapat memicu malnutrisi dan sebaliknya malnutrisi yang berdampak pada defisiensi makronutrien dan mikronutrien juga berpotensi memicu kejang. Seorang anak laki-laki, 14 tahun 6 bulan, dengan status epileptikus disertai status gizi kurang. Kejang berulang menyebabkan pasien tidak sadar sehingga berisiko gagal napas. Terapi multidisiplin oleh bagian anastesi, pediatrik, dan gizi klinik diawali dengan pemasangan ventilator, pemberian medikamentosa antikonsulvan, pemasangan nasogastric tube untuk akses nutrisi dari bagian gizi klinik. Terapi gizi difokuskan pengaturan diet dengan keadaan status epileptikus yang memberikan keadaan kritis pada anak serta perhatian pada kondisi gizi kurang yang memerlukan penanganan jangka panjang. Target pemenuhan diet dicapai pada hari ke 6-8 perawatan melalui diet enteral hingga oral. Tak kalah penting edukasi catch up growth kepada keluarga diberikan untuk dijalankan pada masa pemulihan hingga rehabilitasi di rumah.
